I have chosen to have my child participate in World Relief Programs that provide in-person and/or online engagement with staff and/or adult volunteers of World Relief.

GENERAL INFORMATION:
I understand that, with regard to any of World Relief Programs for minors:
 World Relief staff and/or volunteers will act as adult role models, helpers and sources of friendship and encouragement.
 The activities between my child and the adult volunteer/staff member will be monitored by the Program Manager in charge of the relationship.
 The staff/volunteers that will be connecting with my child have been screened and trained.
 World Relief respects my role as a parent/guardian and will provide an opportunity for me to meet the staff/volunteer (in person or virtually) and be involved in the development of their relationship.
 As my child goes through the program, World Relief will be evaluating satisfaction with the program and the staff/volunteer with which my child is connected.
 All information gathered about the effect of the relationship on my child is strictly for the purposes of evaluating the program and will be kept confidential.
 With regard to any virtual engagement with my child, I understand that World Relief staff/volunteers may contact me or my child to set virtual meetings to facilitate World Relief programs for my child. I understand
that my child may not be home alone for those meetings. World Relief will provide two adult volunteers/staff for any virtual engagement with a child.

In consideration of the ability to participate in any of World Relief’s Programs and further recognizing the voluntary nature of my child’s participation in this event, I, the undersigned person, intending to be legally bound, hereby
promise to waive for myself, my guardians, heirs, executor, administrators, legal representatives and any other persons on my behalf, any and all rights and claims for damages, demands, and any other actions whatsoever, including those attributable to simple negligence, which I or my child may have against World Relief, arising out of
my child’s participation in any of World Relief’s Programs. I FURTHER VERIFY THAT I HAVE FULL KNOWLEDGE OF THE RISKS
ASSOCIATED WITH ATTENDING THIS EVENT. I EXPRESSLY, KNOWINGLY, AND VOLUNTARILY ASSUME THE RISKS INVOLVED IN THE PLANNED ACTIVITIES INCLUDING TRANSPORTATION TO AND FROM ANY WORLD RELIEF PROGRAMS OR RELATED EVENTS, AND AGREE TO HOLD WORLD RELIEF HARMLESS FOR ANY RESULTING INJURY. I understand that this agreement shall remain in effect until the end of my child’s participation in any of World Relief’s Programs. I understand that,
should I decline to execute this agreement, my child will not be permitted to attend the organized event. I have spoken with my child about connecting with a World Relief staff member/volunteer both online and in person.

This form is for any minor (under the age of 18 yrs. old) that has permission from their parent or legal guardian to participate in World Relief Programs, whether through in-person engagement or video conferencing/online
communication platforms.

I give permission for my child/ren listed on this registration form to participate in World Relief programs. I understand the nature and expectations of the program and reserve the right to withdraw my child from the program at any time.

By agreeing, I give my permission for the child(ren) registered above to participate in youth programming with World Relief, including athletic activities and program-related transportation. I understand that there are risks associated with participation in sports and travel, and agree to release and hold harmless World Relief staff and volunteers from any claims of personal injury, illness, or damages of any kind. In the event of an emergency, I give permission to World Relief Western Washington to take necessary emergency measures for the care and protection of my child(ren), while under their supervision. In case of a medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if it is deemed necessary. It is understood that in some medical situations, World Relief staff will need to contact the local emergency resources before the parents/guardians, and/or other adults acting on the parents/guardians behalf are notified. I understand that any expenses incurred will be borne by the child’s family.

For any in-person engagement, I understand that:
 If my child is injured during an in-person World Relief Program and requires the attention of a health care
provider, I hereby consent to, and will be responsible for, any medical treatment as deemed necessary by a
licensed physician.
 I further agree to hold World Relief and its representatives free and harmless of any claims, demands or suits for
damages arising from the authorization and provision of such medical treatment.
